Important Reminders for the 2023-2024 School Year

ILLNESS

Please do not send your child to school if he/she/they present with the following symptoms:

  • Fever or chills in the past 24 hours
  • Vomiting or diarrhea in the past 24 hours
  • Strep Throat – must have been taking antibiotic for at least 24 hours before returning to school.
  • Head Lice - unless they have been treated according to the nurse’s or provider’s instructions.

A child who is sick will not be able to perform well in school and is likely to spread the illness to other children and/or staff. If your child becomes ill while at school, you will be called to pick up your child. It is essential that we have a phone number where you can be reached and an emergency number in the event you cannot be reached. If your daytime or emergency phone number changes during the year, please notify the school immediately.

VACCINATION

Caduceus

All 7th and 12th graders need meningococcal vaccine to attend school. This is a N.Y. State requirement. Check with your child’s health provider to see if they have had it.

PHYSICAL EXAM for all students entering 7, 9 and 11th grade

N.Y. State requires all new students and those students entering grades 7, 9 and 11 to have a physical exam (health appraisal). If your child is a new student or will be entering 7th, 9th or 11th grade in September 2023, you must submit to our office a physical exam that has been done within the past year. We suggest keeping a copy of your child’s physical exam for your own records.

SPORTS PHYSICAL/FAMILY ID

All students wishing to participate in sports must have parent/guardian register them at FamilyID within 30 days of each sport. In addition all athletes must have a current NYS physical exam on file in health office. Current physical exam means less than 1 year old.

MEDICATIONS

A medication order form must be completed by a parent/guardian and your health care provider in order for your child to take, carry or to be given medication by the school nurse. Please note nurses are not permitted to give any medication even OTC medication without a signed MD note. If your child gets frequent headaches, or menstrual discomfort we strongly advise to send completed forms with medication so we can dispense if needed. All medication must be in the original container and clearly labeled. All completed forms should be mailed or faxed (631) 298-3844, directly to the health office.
